Out-of-Network Benefits & Superbill
Even if your insurance plan is not listed above, you may still be eligible for reimbursement through your out-of-network benefits. Many PPO and some HMO plans provide partial coverage for out-of-network physical medicine and rehabilitation services.
After each visit, we can provide a detailed superbill — an itemized receipt that includes all the procedure codes (CPT codes) and diagnosis codes your insurer needs to process a claim. You pay at the time of service, then submit the superbill to your insurance company for reimbursement.

Insurance benefits can vary significantly by plan tier, employer group, and benefit year. We strongly recommend taking these steps before your first appointment:
- Call the member services number on the back of your insurance card and ask about your physical medicine and rehabilitation benefits.
- Confirm whether a referral or prior authorization is required.
- Ask about your deductible, copay, and any visit limits.
