We want your first visit to be smooth and productive. Here is everything you need to know before you arrive.

Our practice is by appointment only. Please call (718) 382-7755 to schedule your visit.

What to Bring

  • Insurance card

    Front and back — we will make a copy at check-in.

  • Photo ID

    Driver's license, passport, or state-issued ID.

  • Referral or prescription (if applicable)

    Required by some insurance plans. Check with your insurer in advance.

  • List of current medications

    Including dosages and frequency, as some medications affect treatment planning.

  • Previous imaging or test results

    MRI, X-ray, or EMG reports if available — these help Dr. Lemberikman review your history.

  • Comfortable, loose-fitting clothing

    We may need to observe your movement or access the area being evaluated.

What to Expect at Your First Visit

Your initial evaluation is typically scheduled for 45-60 minutes. Here is how that time is structured:

Intake & Medical History

Dr. Lemberikman will review your health history, ask about your symptoms, and listen to what has worked — or hasn't — before. Plan for about 15 minutes of conversation.

Physical & Neurological Evaluation

A thorough assessment of your strength, range of motion, reflexes, and movement patterns. This is where we identify the root cause, not just the symptoms.

Findings & Treatment Plan

Dr. Lemberikman will explain what she found in plain language and outline a proposed treatment plan, including any recommended diagnostics, therapy frequency, and expected timeline.

Initial Treatment or Diagnostic Order

Depending on your condition, treatment may begin the same day, or Dr. Lemberikman may order electrodiagnostic testing or imaging to confirm the diagnosis first.

Please arrive 10 minutes early for your first appointment to complete any remaining paperwork and get settled.

Cancellation Policy

We understand that life is unpredictable. If you need to reschedule or cancel your appointment, we ask for at least 24 hours' notice so we can offer that time to another patient.

Cancellations made with less than 24 hours' notice, or no-shows, may be subject to a late cancellation fee. This fee is not billable to insurance.

Common Questions

Do I need a doctor's referral?
Some insurance plans require a referral for PM&R services. We recommend checking with your insurer before your first visit. If you are unsure, call our office and we can help you find out.
How many visits will I need?
This depends on your diagnosis, goals, and how your body responds to treatment. Dr. Lemberikman will give you a realistic estimate after your initial evaluation.
